FAA Releases Diesel Exhaust Fluid (DEF) Safety Risk Assessment Report

The Federal Aviation Administration (FAA) released a safety risk assessment report on jet fuel contamination with Diesel Exhaust Fluid (DEF). The assessment was based on the risk matrix and severity and likelihood definitions from the FAA Order 8040.4B, Safety Risk Management Policy. The report was prepared by the Safety Risk Management (SRM) team comprised of NATA staff, FAA representatives, and industry stakeholders. As noted in the memorandum, the SRM team identified a high initial risk associated with this safety issue and the potential to have a high residual risk, due to the common cause factor that the fuel system supplies all engines on the aircraft of failure, if joint efforts with the Environmental Protection Agency (EPA) are unable to remove the hazard. The FAA continues to work with the EPA on potential regulatory mitigations.

The report outlines the background/overview of the DEF contamination events, the safety risk management planning and methodology, system analysis/description, the safety risk analysis and assessment, provides 15 safety recommendations to mitigate the risk of jet fuel contamination with DEF, and details the FAA’s tracking and monitoring plan for this assessment.

NATA sent out free Diesel Exhaust Fluid (DEF) decals to its members that can be used to clearly label storage containers and transfer equipment specifically for DEF use to avoid future contamination emergencies. Available upon request to Safety1st@nata.aero.

With the Jan. 1, 2020 deadline looming for aircraft flying in most U.S. airspace to comply with the ADS-B Out mandate, business aircraft service provider Duncan Aviation recommends operators perform an emitter status check to ensure their equipment is compliant. “A recent report found there are more than 7,700 aircraft with non-performing emitters (NPEs),” said Nate Klenke, Duncan Aviation manager of completions and modification services sales. “Many of the aircraft with NPEs were likely installed more than four years ago or prior to changes made in the mandate. Some may be a result of an improper installation or equipment configuration.” Read more.
